TRT Monitoring Panel: Blood Work for Men on Testosterone Therapy
If you are on testosterone therapy, your clinician needs the same few numbers checked again and again: your testosterone level, your hematocrit and your PSA. This panel puts that TRT blood work in one fasting draw, with estradiol, liver and kidney function and cholesterol alongside.
Testosterone therapy is monitored with blood work, not by feel alone. The Endocrine Society's clinical practice guideline recommends checking testosterone levels 3 to 6 months after starting treatment, checking hematocrit at baseline, at 3 to 6 months and then every year, and, for men who choose prostate monitoring, checking PSA 3 to 12 months after starting. This panel gathers those labs, plus a few general health markers, so one visit covers them.
Free and total testosterone (measured by mass spectrometry, with free testosterone by equilibrium dialysis) show where your level sits on treatment. Free testosterone matters most when SHBG, the protein that carries testosterone, is unusually high or low.
Hematocrit, part of the complete blood count, is the safety marker the guideline watches most closely: testosterone can raise red blood cell levels, and a hematocrit above 54% is a reason for the prescribing clinician to reassess therapy.
PSA tracks the prostate. The guideline suggests urology referral if PSA rises more than 1.4 ng/mL within the first 12 months of treatment, or is confirmed above 4.0 ng/mL.
Sensitive estradiol (LC/MS) is designed to measure the low estradiol levels found in men, and gives your clinician a second hormone to read alongside testosterone. The comprehensive metabolic panel and lipid panel add liver and kidney function, glucose and cholesterol, the general markers many clinicians review at the same visit.
JustLabs does not prescribe testosterone or advise on doses. This panel is for monitoring: share the results with the clinician who manages your treatment.

Included Tests
Testosterone, Free & Total
Comprehensive testosterone panel measuring both free and total testosterone levels.
Estradiol, Ultrasensitive, LC/MS
Highly sensitive estradiol measurement using liquid chromatography/tandem mass spectrometry (LC/MS). Essential for detecting low estradiol levels in men, children, and postmenopausal women.
Complete Blood Count with Differential
Comprehensive blood cell analysis including red cells, white cells, platelets, and white cell differential.
Comprehensive Metabolic Panel
Measures 14 substances including glucose, electrolytes, kidney and liver function markers.
Fasting
Lipid Panel (Standard)
Standard cholesterol panel including total cholesterol, HDL, LDL, and triglycerides. Essential for cardiovascular risk assessment.
PSA, Total
Prostate-Specific Antigen test for prostate cancer screening. Recommended for men over 50 (or 40 with risk factors).
Fasting Required
One or more tests in this panel require fasting for accurate results:
- Do not eat or drink anything except water for 8-12 hours before your blood draw.
- Water is allowed and encouraged - stay hydrated!
- Schedule your lab visit in the morning for convenience.
What This Panel Measures
Free testosterone (equilibrium dialysis) and total testosterone (mass spectrometry); estradiol by an ultrasensitive LC/MS method designed for the low levels found in men; a complete blood count with differential, including hemoglobin and hematocrit; total PSA; a comprehensive metabolic panel (glucose, kidney function with eGFR, electrolytes, calcium, protein, albumin, bilirubin and the liver enzymes ALT, AST and alkaline phosphatase); and a standard lipid panel (total cholesterol, LDL, HDL, non-HDL cholesterol and triglycerides).
Who Should Get This Panel
Adult men who are already on testosterone therapy prescribed by a clinician and need their routine monitoring labs, for example at the 3-to-6-month check after starting or changing treatment, or at the yearly check. It is also useful for men who want a complete set of results to bring to a new prescriber. If you have not started testosterone and want to know whether your level is low, the TRT Baseline Panel is the better fit. Available to adults 18 and over only.

When to Get Tested
The Endocrine Society guideline recommends checking testosterone 3 to 6 months after starting therapy, hematocrit at baseline, at 3 to 6 months and then yearly, and PSA 3 to 12 months after starting for men who choose prostate monitoring; after the first year, prostate monitoring follows the usual screening guidance for your age and risk. When to draw relative to your dose depends on the form of testosterone, and the guideline gives formulation-specific timing (for example, midway between injections for testosterone cypionate or enanthate, and 2 to 8 hours after applying a gel). Ask the clinician who prescribes your treatment which timing they want, and book a morning appointment after an 8- to 12-hour fast.
Understanding Your Results
Each result is read against Quest's reference range and against your own earlier results. Testosterone well above or below the range your clinician is aiming for, a hematocrit that has climbed (the guideline treats above 54% as a reason to reassess therapy), or a PSA that has risen more than 1.4 ng/mL in the first year of treatment or is confirmed above 4.0 ng/mL are the findings most likely to prompt a change of plan or a referral. A high or low estradiol is interpreted together with testosterone and any symptoms. Raised liver enzymes, reduced kidney function, high glucose or unfavorable cholesterol are general health findings that your clinician weighs alongside your treatment. Do not change your dose or stop treatment based on these results yourself. JustLabs results are informational: review them with the clinician who manages your testosterone therapy.
