TRT Baseline Panel: Testosterone Blood Test and Pre-Treatment Labs
A testosterone blood test answers one question: is the number low? Before anyone starts testosterone therapy, clinicians also want to know why it is low and whether it is safe to treat. This panel pairs a morning free and total testosterone test with the hormones that reveal the cause and the baseline safety labs, in one fasting draw.
The Endocrine Society's guideline says low testosterone should be diagnosed only when a man has symptoms and consistently low levels, measured on a fasting morning sample and confirmed on a second morning. A single testosterone number is a starting point, not a diagnosis. This panel is built around the questions a clinician asks before testosterone therapy is considered.
Is testosterone really low? Free and total testosterone are measured by mass spectrometry, with free testosterone by equilibrium dialysis, the method the guideline prefers. SHBG is included because when it is high or low (for example with obesity), total testosterone can mislead and free testosterone tells the real story.
Why is it low? LH and FSH separate a problem in the testes (primary hypogonadism, high LH and FSH) from a problem in the pituitary or hypothalamus (secondary, low or normal LH and FSH). Prolactin is the guideline's next step for secondary causes, since a high prolactin can suppress testosterone. TSH screens for thyroid disease, which can cause overlapping symptoms such as fatigue and low libido.
Is it safe to start? The guideline lists an elevated hematocrit and a raised PSA among the reasons to pause and evaluate before treatment, so a blood count and PSA establish your baseline. Sensitive estradiol, an A1c, a metabolic panel and cholesterol give your clinician a starting picture to compare against later.
Compared with the Male Hormone Panel, which looks at testosterone alongside DHEA-S and cortisol, this panel adds the cause-finding hormones and the safety baselines needed before treatment. JustLabs does not prescribe testosterone; take your results to a clinician.

Included Tests
Testosterone, Free & Total
Comprehensive testosterone panel measuring both free and total testosterone levels.
Sex Hormone Binding Globulin
Measures SHBG which binds sex hormones. Affects free hormone availability.
Estradiol, Ultrasensitive, LC/MS
Highly sensitive estradiol measurement using liquid chromatography/tandem mass spectrometry (LC/MS). Essential for detecting low estradiol levels in men, children, and postmenopausal women.
FSH & LH
Combined panel measuring both FSH and LH for comprehensive reproductive hormone assessment.
Prolactin
Measures prolactin hormone. Elevated levels can affect fertility and menstrual cycles.
Fasting
Complete Blood Count with Differential
Comprehensive blood cell analysis including red cells, white cells, platelets, and white cell differential.
Comprehensive Metabolic Panel
Measures 14 substances including glucose, electrolytes, kidney and liver function markers.
Fasting
Lipid Panel (Standard)
Standard cholesterol panel including total cholesterol, HDL, LDL, and triglycerides. Essential for cardiovascular risk assessment.
Hemoglobin A1c (HbA1c)
Measures average blood sugar over the past 2-3 months. Key marker for diabetes management and prediabetes screening.
PSA, Total
Prostate-Specific Antigen test for prostate cancer screening. Recommended for men over 50 (or 40 with risk factors).
TSH (Thyroid Stimulating Hormone)
Measures thyroid-stimulating hormone to evaluate thyroid function. TSH is the most sensitive marker for detecting thyroid disorders.
Fasting Required
One or more tests in this panel require fasting for accurate results:
- Do not eat or drink anything except water for 8-12 hours before your blood draw.
- Water is allowed and encouraged - stay hydrated!
- Schedule your lab visit in the morning for convenience.
What This Panel Measures
Free testosterone (equilibrium dialysis) and total testosterone (mass spectrometry); sex hormone binding globulin (SHBG); estradiol by ultrasensitive LC/MS; luteinizing hormone (LH) and follicle-stimulating hormone (FSH); prolactin; TSH; a complete blood count with differential, including hematocrit; total PSA; hemoglobin A1c; a comprehensive metabolic panel (glucose, kidney function, electrolytes, liver enzymes, albumin, bilirubin) and a standard lipid panel.
Who Should Get This Panel
Adult men with symptoms that can come with low testosterone, such as reduced sex drive, erectile problems, low energy, reduced muscle or loss of body hair, who want a complete evaluation; men who had one low testosterone result and need a confirming morning test along with the cause-finding hormones; and men who are discussing testosterone therapy with a clinician and need their pre-treatment labs. The Endocrine Society recommends against routinely screening men without symptoms. Men planning to father a child soon should know that testosterone therapy can reduce sperm production; discuss this with a clinician. Available to adults 18 and over only.

When to Get Tested
Test when you have symptoms, not as routine screening. Book an early-morning appointment, ideally between 7 and 10 a.m., when testosterone is usually highest, after an 8- to 12-hour fast. The guideline recommends confirming a low result on a second morning, so a repeat testosterone test is common. Avoid testing during an acute illness or while taking a short course of a medicine that suppresses testosterone, such as opioids, since results will not reflect your usual level. If you are already on testosterone therapy, the TRT Monitoring Panel is the right choice instead.
Understanding Your Results
Results are read together. A normal free and total testosterone makes low testosterone unlikely as the cause of symptoms. A low result is usually repeated on another morning before any diagnosis. If testosterone is low, high LH and FSH point to a testicular cause, while low or normal LH and FSH point to the pituitary or hypothalamus; in that case a high prolactin, or a very low testosterone, may lead your clinician to order more tests or pituitary imaging. An abnormal TSH suggests a thyroid problem that may explain symptoms. A high hematocrit or a raised PSA is usually evaluated before testosterone therapy is considered. A1c, glucose, liver and kidney function and cholesterol are general health findings and form the baseline for later monitoring. JustLabs results are informational: review them with a clinician, who makes the diagnosis and decides whether treatment is appropriate.
