Women's Health Blood Test Panel: Annual Check with Iron, Thyroid & B12

A yearly blood test for women that covers the usual checkup basics (blood count, kidney and liver function, cholesterol and blood sugar) and adds the four things that most often explain tiredness in women: iron stores, thyroid function, vitamin D and vitamin B12. Nine tests, one fasting draw at Quest.

Most annual blood work checks a blood count, a metabolic panel and cholesterol. That is a good start, but it can miss the causes of the symptoms women bring up most often, like tiredness, feeling cold, hair shedding or brain fog. This women's blood panel keeps the standard checkup tests and adds the markers that cover those gaps.

Iron stores (ferritin). Heavy, long or frequent periods and pregnancy are common causes of iron loss, according to MedlinePlus. Ferritin reflects your iron reserves, and the complete blood count shows whether low iron has gone on to cause anemia. Read together, they can show iron running low before anemia appears.

Thyroid (TSH and free T4). NIDDK notes that women are much more likely than men to develop an underactive thyroid, whose symptoms include fatigue, weight gain, feeling cold and heavy or irregular periods. TSH is the screening test; free T4 shows how much active hormone is circulating, which helps interpret a TSH that is borderline.

Vitamin B12 and vitamin D. B12 deficiency is more likely with a strict vegetarian diet, long-term metformin or heartburn medicines, weight-loss surgery or pernicious anemia (MedlinePlus). Vitamin D helps the body absorb calcium for strong bones.

The checkup basics. The CBC, the comprehensive metabolic panel (glucose, kidney and liver function, electrolytes), the lipid panel and hemoglobin A1c cover blood counts, organ function, heart risk and blood sugar. The USPSTF recommends prediabetes screening for adults 35 to 70 with overweight or obesity.

How it compares: our Comprehensive Wellness Panel has the same core but includes hs-CRP instead of vitamin B12; our Energy & Fatigue Panel focuses on symptoms and leaves out cholesterol and A1c. Choose this one if you want a yearly check built around women's most common deficiencies. It is $85, $6.50 less than the nine tests ordered separately.

What This Panel Measures

Nine tests: complete blood count with differential (red cells, hemoglobin, hematocrit, white cells and platelets), comprehensive metabolic panel (glucose, kidney function with eGFR, liver enzymes, electrolytes, calcium, protein), standard lipid panel (total, LDL, HDL and non-HDL cholesterol, triglycerides), hemoglobin A1c, TSH, free T4, ferritin (iron stores), 25-hydroxy vitamin D and vitamin B12.

Who Should Get This Panel

Adult women who want a yearly checkup-style blood test they can order themselves; women with heavy or long periods, who are vegetarian or vegan, or who take metformin or acid-reducing medicine and want iron and B12 checked; women with tiredness, feeling cold, hair shedding or brain fog who want the common causes looked at together; and women with a family history of thyroid disease, diabetes or high cholesterol.

When to Get Tested

Once a year as part of routine health maintenance, or sooner if you have new symptoms such as ongoing fatigue, heavier periods, hair shedding or feeling cold. If you are being treated for low iron, low B12, low vitamin D or a thyroid condition, your clinician may suggest repeating the relevant tests to see how you are responding. Book a morning appointment after an 8-hour fast.

Understanding Your Results

Each result is reported against Quest's reference range and flagged if high or low, but several are best read together. Low ferritin means iron stores are depleted; with low hemoglobin or small red cells on the CBC it points to iron-deficiency anemia. Ferritin can be falsely normal or high during inflammation, infection or liver disease. A high TSH with low free T4 suggests an underactive thyroid; a high TSH with normal free T4 is a milder pattern that clinicians often recheck. Low B12, especially with large red cells on the CBC, may need follow-up tests such as methylmalonic acid. Low vitamin D, high LDL or triglycerides, an A1c in the prediabetes or diabetes range, or abnormal kidney or liver values on the CMP each have their own next steps. JustLabs results are informational: review them with a clinician, who can confirm a finding and decide on treatment.
