Hair Loss Blood Test Panel (Iron, Ferritin, Thyroid, Vitamin D, B12 & Androgens)

Hair loss has many causes, and some of them show up in your blood. This hair loss blood test checks the common, treatable ones a clinician looks for first: low iron stores, an overactive or underactive thyroid, low vitamin D or B12, anemia, and, especially in women, too much androgen. It can't diagnose hereditary hair loss, but it can rule contributors in or out.

Finding the cause is the first step in treating hair loss, and dermatologists start with your history and a close look at your scalp. When they suspect a disease, a vitamin deficiency or a hormone imbalance, they turn to blood tests. This panel gathers the ones most often used for that purpose into a single draw.

Iron stores and the blood count. Too little iron can cause noticeable shedding. Ferritin shows your stored iron, the iron panel shows what is circulating, and the CBC shows whether low iron has already led to anemia.

Thyroid. Thyroid disease can thin the hair, and the American Academy of Dermatology notes that treating it can reverse the hair loss. TSH and free T4 together screen for an underactive or overactive thyroid.

Vitamin D and B12. Low B12 can contribute to thinning hair, and dermatologists evaluating patchy hair loss (alopecia areata) often check iron and vitamin levels. Both are common, easy-to-correct deficiencies.

Androgens. Free and total testosterone and DHEA-sulfate look for androgen excess, a cause of thinning on the top of the scalp in women, often from polycystic ovary syndrome (PCOS) or an adrenal condition. In men, they add context but cannot diagnose male-pattern baldness.

Be realistic about what to expect. Hereditary hair loss (androgenetic alopecia) is driven by inherited genes, so blood tests are often completely normal; shedding a few months after childbirth, illness or major stress usually settles once the trigger passes. Normal results still help: they rule out treatable contributors. Bring them to a dermatologist, who makes the diagnosis. Zinc is not part of this panel; it can be added as a separate test.

Fasting Required

One or more tests in this panel require fasting for accurate results:

  • Do not eat or drink anything except water for 8-12 hours before your blood draw.
  • Water is allowed and encouraged - stay hydrated!
  • Schedule your lab visit in the morning for convenience.

What This Panel Measures

Nine tests: a complete blood count with differential (anemia and red cell indices); ferritin (stored iron); serum iron, total iron binding capacity and transferrin saturation; TSH and free T4 (thyroid function); 25-hydroxy vitamin D; vitamin B12; free and total testosterone by mass spectrometry and equilibrium dialysis; and DHEA-sulfate (an androgen made mainly by the adrenal glands).

Who Should Get This Panel

Women and men with new or increased shedding, a widening part, or thinning on the top of the scalp; women with thinning hair alongside irregular periods, acne or extra facial or body hair, which can suggest androgen excess or PCOS; people with heavy periods, a vegetarian or vegan diet, or past bariatric surgery who may be low in iron or B12; people with known or suspected thyroid disease; and anyone whose dermatologist or clinician wants these baseline labs.

When to Get Tested

When shedding or thinning is new, sudden or getting worse, ideally before starting supplements so the results reflect your baseline, and before or alongside a dermatology visit. Because thyroid and iron problems are treatable, earlier is better. Book a morning appointment, ideally between 7 and 10 a.m., after a 12-hour fast: iron requires a fasting morning sample, and testosterone is usually measured in the morning when it is highest.

Understanding Your Results

Each result answers a different question. Low ferritin, with or without low iron, saturation or hemoglobin, points to low iron stores, a recognized and treatable contributor to shedding; the next step is finding why iron is low. A high or low TSH, read with free T4, suggests an underactive or overactive thyroid that needs a clinician's evaluation. Low vitamin D or B12 can be corrected and is worth treating whether or not it is the cause of your hair loss. In women, high testosterone or DHEA-S suggests androgen excess and usually leads to further evaluation for PCOS or an adrenal cause. If everything is normal, a blood-based cause is less likely, and the most common explanations are hereditary hair loss or temporary shedding after a stressor; a dermatologist diagnoses these by examining the scalp. JustLabs results are informational: review them with a clinician or dermatologist before starting any treatment or supplement.
