Gout Blood Test Panel: Uric Acid Test, Kidney Function & Inflammation Markers
Gout happens when uric acid builds up and forms crystals in a joint. This panel pairs a uric acid test with the kidney function panel and inflammation markers clinicians look at alongside it, because uric acid on its own can mislead in both directions.
Gout is a form of arthritis caused by urate (uric acid) crystals in a joint. A flare usually starts suddenly, often at night, with intense pain, swelling, redness and warmth, frequently in the big toe, and typically lasts a week or two. Uric acid builds up when the body makes too much or the kidneys don't clear enough, and risk is higher with chronic kidney disease, high blood pressure, excess weight, alcohol, sugar-sweetened drinks and some medicines such as diuretics.
Uric acid is the core measurement. But a gout blood test has two well-known limits. Many people have high uric acid and never develop gout. And during a flare, uric acid can fall: New Zealand primary-care guidance notes it reads in the normal range in up to 40% of flares, so a normal result in the middle of an attack does not rule gout out and is usually rechecked once the flare has settled.
Kidney function (Renal Function Panel) is included because the kidneys are how the body clears uric acid. The panel reports creatinine, eGFR, BUN, electrolytes, calcium, phosphorus and albumin, so you and your clinician can see whether kidney function is part of the picture.
hs-CRP and sed rate (ESR) show how much inflammation is present. They cannot say what is causing it, but they help a clinician gauge whether joint pain is inflammatory.
Gout itself is diagnosed by a clinician: the definitive test is finding urate crystals in fluid drawn from the joint, and imaging can help. For people already diagnosed, the 2020 American College of Rheumatology guideline uses repeated serum urate measurements with a target below 6 mg/dL, which makes this panel a practical way to track your level between visits.

Included Tests
Uric Acid
Measures uric acid levels. Elevated levels can indicate gout risk or kidney issues.
Renal Function Panel
Kidney function assessment including BUN, creatinine, and electrolytes.
High-Sensitivity CRP
Measures inflammation marker associated with cardiovascular disease risk. Low-grade inflammation indicator.
ESR (Sed Rate)
Erythrocyte Sedimentation Rate measures inflammation. Elevated in infections, autoimmune diseases, and cancer.

What This Panel Measures
Four tests on one blood sample: serum uric acid; a Renal Function Panel (creatinine, eGFR, BUN, BUN/creatinine ratio, sodium, potassium, chloride, carbon dioxide, calcium, phosphorus and albumin); high-sensitivity C-reactive protein (hs-CRP), a liver-made protein that rises with inflammation; and the erythrocyte sedimentation rate (ESR or sed rate), a second, slower-moving marker of inflammation.
Who Should Get This Panel
People with sudden attacks of joint pain and swelling, especially in the big toe, who are being evaluated for gout; people with a previous high uric acid result; people diagnosed with gout whose clinician tracks their uric acid level over time; and people with gout risk factors such as chronic kidney disease, high blood pressure, metabolic syndrome, excess weight, a family history of gout, or medicines known to raise uric acid, such as diuretics or low-dose aspirin.

When to Get Tested
After a suspected gout attack, ideally once the flare has settled, because uric acid can read lower during a flare. If you test during a flare, the inflammation markers are still informative, but a normal uric acid result should be repeated later. If you have been diagnosed with gout, test at the interval your clinician sets for checking your uric acid against the guideline target. There is no need to fast.
Understanding Your Results
Results are read together. A high uric acid supports a gout diagnosis in someone with typical attacks, but it is common in people who never get gout and is not a diagnosis on its own. A normal uric acid measured during a flare does not rule gout out and is usually repeated once the flare has passed. High hs-CRP and sed rate show inflammation without identifying its cause; gout, infection and other forms of arthritis can all raise them. A low eGFR or high creatinine suggests reduced kidney function, which can raise uric acid and is important for your clinician to know. For people already treated for gout, the ACR guideline compares serum urate with a target below 6 mg/dL; how to reach it is a decision for you and your clinician. JustLabs results are informational: review them with a clinician, who may examine fluid from the affected joint for crystals or order an ultrasound or CT scan.
