Fatty Liver Blood Test Panel (MASLD, formerly NAFLD): Liver Enzymes, FIB-4 Inputs & Metabolic Markers
Fatty liver disease, now called MASLD (formerly NAFLD), usually shows up first in routine blood work. This panel pairs a liver enzyme test with the metabolic markers that drive fatty liver, and includes every lab value your clinician needs to calculate a FIB-4 score, the first step guidelines use to judge whether the liver has started to scar.
Fatty liver happens when extra fat builds up in the liver. In 2023 the condition was renamed metabolic dysfunction-associated steatotic liver disease (MASLD), replacing nonalcoholic fatty liver disease (NAFLD), to reflect what drives it: excess weight, high blood sugar, high blood pressure and abnormal blood fats. The NIDDK estimates that about 24% of US adults have it, and it is far more common with obesity and type 2 diabetes. In some people the fat comes with inflammation (MASH, formerly NASH), which can lead to scarring (fibrosis) and, over years, cirrhosis.
Liver enzymes (CMP and GGT). The Comprehensive Metabolic Panel reports ALT, AST, alkaline phosphatase, bilirubin and albumin. GGT adds a second view: it helps tell a liver cause of a high alkaline phosphatase from a bone cause, and it rises with alcohol, which matters because heavier drinking changes the picture (the MetALD category).
Platelets (CBC). The platelet count is one of the four inputs to FIB-4, alongside age, AST and ALT.
The metabolic drivers (lipid panel, A1c, fasting insulin). MASLD is defined by at least one cardiometabolic risk factor, such as an A1c of 5.7% or higher, triglycerides of 150 mg/dL or higher, or low HDL. Fasting insulin, read alongside the fasting glucose in the CMP, can point to insulin resistance.
FIB-4. AASLD guidance uses FIB-4, a score built from your age, AST, ALT and platelet count, as the first step in sorting low risk of advanced fibrosis from those who need a second test such as elastography (FibroScan). This panel gives your clinician all the lab inputs; it does not diagnose fatty liver, which is confirmed with imaging.

Included Tests
Comprehensive Metabolic Panel
Measures 14 substances including glucose, electrolytes, kidney and liver function markers.
Fasting
GGT (Gamma-Glutamyl Transferase)
Liver enzyme that can indicate liver disease or bile duct problems. Also elevated with alcohol use.
Complete Blood Count with Differential
Comprehensive blood cell analysis including red cells, white cells, platelets, and white cell differential.
Lipid Panel (Standard)
Standard cholesterol panel including total cholesterol, HDL, LDL, and triglycerides. Essential for cardiovascular risk assessment.
Hemoglobin A1c (HbA1c)
Measures average blood sugar over the past 2-3 months. Key marker for diabetes management and prediabetes screening.
Insulin, Fasting
Measures fasting insulin levels to assess insulin resistance and metabolic health. Important for early detection of diabetes risk.
Fasting
Fasting Required
One or more tests in this panel require fasting for accurate results:
- Do not eat or drink anything except water for 8-12 hours before your blood draw.
- Water is allowed and encouraged - stay hydrated!
- Schedule your lab visit in the morning for convenience.
What This Panel Measures
Six tests from one fasting blood draw: the Comprehensive Metabolic Panel (ALT, AST, alkaline phosphatase, bilirubin, albumin, total protein, fasting glucose, kidney function and electrolytes), GGT (gamma-glutamyl transferase, a liver and bile-duct enzyme that also rises with alcohol), a CBC with differential (including the platelet count used in FIB-4), a standard lipid panel (total cholesterol, LDL, HDL, triglycerides, non-HDL), hemoglobin A1c (average blood sugar over about three months) and fasting insulin.
Who Should Get This Panel
Adults with overweight or obesity, prediabetes or type 2 diabetes, high triglycerides or low HDL, or high blood pressure, since these are the risk factors that define MASLD; people told on an ultrasound or CT that they have a fatty liver who want their liver enzymes and FIB-4 inputs checked; people with a previous high ALT or AST that was never followed up; and anyone a clinician has asked to repeat liver and metabolic labs to track changes over time.

When to Get Tested
When you have one or more metabolic risk factors and have not had liver enzymes and a platelet count checked recently, after an imaging report mentions fatty liver or steatosis, or when a clinician wants to repeat FIB-4 inputs to follow changes over time. Book a morning appointment after a 10- to 12-hour fast. Recent illness can temporarily change liver test results, so if you have just been sick, consider waiting until you have recovered.
Understanding Your Results
Results are read together. A high ALT or AST shows the liver is under stress but does not say why: fatty liver is a common cause, and alcohol, medicines, viral hepatitis and other conditions are others. A high GGT with a high alkaline phosphatase points to the liver or bile ducts rather than bone. A low platelet count is worth reviewing with a clinician, since it is part of the FIB-4 score. An A1c of 5.7% or higher, triglycerides of 150 mg/dL or higher, low HDL or a high fasting insulin with normal glucose describe the metabolic pattern behind MASLD. Your clinician can use your age, AST, ALT and platelets to calculate FIB-4: under AASLD guidance a low score means advanced fibrosis is unlikely, and a higher score usually leads to elastography (such as FibroScan) or another noninvasive test. Normal results do not rule out fatty liver, which is seen on imaging. JustLabs results are informational: review them with a clinician, who may order an ultrasound, elastography or tests for other causes such as hepatitis B and C.
