Total Testosterone Test (LC/MS/MS)

Total Testosterone

Measures the total amount of testosterone in your blood using liquid chromatography with tandem mass spectrometry (LC/MS/MS). It is used to look into symptoms of low testosterone in men, signs of high testosterone in women, and to monitor hormone therapy.

Testosterone is the main male sex hormone, but everyone makes it: the testes produce most of it in men, and the ovaries and adrenal glands produce smaller amounts in women. It influences sex drive, muscle and bone, red blood cell production, body hair and, in women, menstrual regularity.

Total testosterone includes both the testosterone bound to proteins in the blood and the small free fraction. It is the standard first test. Levels are usually highest in the morning, so clinicians generally test between about 7 and 10 a.m.

This test uses LC/MS/MS rather than a standard immunoassay. That matters most when levels are low, as they normally are in women and children and can be in men with low testosterone. Proficiency testing coordinated by the CDC has found that some direct immunoassays are inaccurate at low testosterone concentrations, while mass spectrometry separates testosterone from similar molecules before measuring it.

If your total testosterone is borderline, or you have a condition that changes sex hormone binding globulin (SHBG), your clinician may want free testosterone as well. JustLabs offers the Testosterone, Free & Total test and an SHBG test for that purpose.

What This Test Measures

Total testosterone in serum: the hormone bound to sex hormone binding globulin (SHBG) and albumin plus the small unbound (free) fraction. Quest measures it by chromatography/mass spectrometry (LC/MS/MS), which isolates testosterone from other steroids before measuring it.

Who Should Consider This Test

Men with symptoms that may suggest low testosterone, such as low sex drive, erectile dysfunction, reduced body hair or loss of muscle mass; women with signs of high testosterone, such as excess facial or body hair, acne, or irregular or absent periods; and people on testosterone or other hormone therapy whose clinician wants levels monitored. Children and teens are tested when puberty seems early or delayed, under a clinician's care.

When to Get Tested

In the morning, ideally between about 7 and 10 a.m., when testosterone is usually highest. When low testosterone is being evaluated in men, the Endocrine Society recommends a fasting morning sample and confirming a low result with a repeat morning fasting measurement. If you are on testosterone therapy, follow your prescriber's advice on when to test relative to your dose.

Understanding Your Results

Quest reports total testosterone in ng/dL against age- and sex-specific reference ranges; for adults 18 and older, Quest's ranges for this test are 250-1100 ng/dL for males and 2-45 ng/dL for females. In men, a low result can reflect problems with the testes or pituitary gland, chronic illness, certain medications or alcohol use, and levels naturally decline with age; one low value should be confirmed with a repeat morning test. In women, a high result can be seen with polycystic ovary syndrome (PCOS), adrenal conditions such as congenital adrenal hyperplasia, or, rarely, ovarian or adrenal tumors. A testosterone result alone does not make a diagnosis: review it with a clinician, who will consider your symptoms and may add free testosterone, SHBG, LH or other tests.
