Order Parietal Cell Antibody Test with Reflex to Titer from JustLabs for $41 ($59 all-in for a single-test order: $41 + one $18 lab & physician order fee per order). Price includes lab processing at Quest Diagnostics. No insurance required.
Parietal cell antibodies target the cells that make stomach acid and intrinsic factor. Quest reports them in about 80% of people with pernicious anemia, which makes this a sensitive first look, though they also turn up in some healthy people. A positive screen is automatically followed by a titer on the same sample.
Parietal cells line the upper part of the stomach. They make stomach acid and intrinsic factor, the protein needed to absorb vitamin B12. In autoimmune gastritis, the immune system targets these cells, and this test looks for the antibodies involved.
As parietal cells are lost, stomach acid falls. Low acid makes iron harder to absorb, so iron deficiency is common, and as intrinsic factor declines, vitamin B12 deficiency can follow. Pernicious anemia is the late stage of this process. Many people have no symptoms for years.
The test works in two steps. First, a screen checks whether parietal cell antibodies are present. If the screen is positive, the lab automatically runs a titer on the same blood sample, which shows how strongly positive the result is. You do not need to order the titer separately or come back for a second draw.
Parietal cell antibodies are a sensitive marker: Quest reports them in about 80% of people with pernicious anemia. The trade-off is specificity. Quest notes they can appear in up to 10% of healthy people and in other autoimmune conditions, so a positive result is a reason to look further, not a diagnosis. That is why this test is most useful alongside intrinsic factor antibodies, which are less sensitive but highly specific, and gastrin, which rises when the stomach makes little acid. It is also worth considering if you have Hashimoto's, because thyroid and stomach autoimmunity frequently occur together.

What's Measured
What This Test Measures
This test screens serum for antibodies against gastric parietal cells, using an immunofluorescence assay (IFA) at Quest. The expected result is negative. If the screen is positive, the lab automatically performs a parietal cell antibody titer on the same sample, which shows how strongly positive the result is.
Who Should Consider This Test
People with iron deficiency or vitamin B12 deficiency that has no clear explanation; people with Hashimoto's thyroiditis, type 1 diabetes or another autoimmune condition who want to check for stomach autoimmunity; people with a family history of pernicious anemia; and anyone a clinician is evaluating for autoimmune gastritis.

When to Get Tested
Any time; this test needs no special timing or fasting. It is most informative when you are investigating low B12, low ferritin or iron, or anemia without an obvious cause, or as part of a check for other autoimmune conditions after an autoimmune thyroid diagnosis.

Understanding Your Results
Negative means parietal cell antibodies were not detected, which makes autoimmune gastritis less likely, though a clinician may still investigate if symptoms or other results point that way. Positive means antibodies are present, and your report will also include a titer showing how strongly positive the result is. Because these antibodies also occur in some healthy people and in other autoimmune disorders, a positive result is not a diagnosis on its own, whatever the titer. It is read together with gastrin, intrinsic factor antibodies, B12, iron studies and a blood count, and confirming autoimmune gastritis usually involves a gastroenterologist and stomach biopsies taken during an endoscopy. JustLabs results are informational; review them with a clinician.
What to Expect
After placing your order, we send it straight to Quest Diagnostics — there is no consent form to sign and no partner email to wait for. Your requisition is usually ready to download from your dashboard within about an hour, and you can book your blood draw at any Quest Diagnostics location.
