Autoimmune Gastritis & Pernicious Anemia Blood Test Panel
Autoimmune gastritis is a slow immune attack on the stomach that can cause iron and vitamin B12 deficiency long before anyone suspects the stomach. This panel combines the three blood markers clinicians use to evaluate it, because each one covers a weakness of the others.
Autoimmune gastritis happens when the immune system targets the parietal cells in the upper stomach. These cells make stomach acid and intrinsic factor, the protein needed to absorb vitamin B12. As they are lost, acid falls, iron becomes harder to absorb and, over time, B12 absorption fails. Iron deficiency anemia is common and can appear first; pernicious anemia, the B12-deficiency anemia caused by this process, is the late stage. Many people have no symptoms for years.
No single blood test settles the question, which is why this panel combines three.
Parietal cell antibodies are the sensitive marker: Quest reports them in about 80% of people with pernicious anemia. But they also appear in up to 10% of healthy people, so on their own they can mislead. If the screen is positive, the lab automatically runs a titer on the same sample to show how strongly positive it is, with no second draw.
Intrinsic factor blocking antibodies are the opposite. They are found in only about half of people with pernicious anemia, but they are very specific: a positive result strongly supports the diagnosis.
Gastrin shows the functional effect. When parietal cells are lost, the stomach keeps signaling for acid it cannot make, so fasting gastrin rises. High gastrin alongside positive antibodies is the classic pattern.
Autoimmune gastritis frequently travels with autoimmune thyroid disease: a review in Acta Biomedica reports that Hashimoto's thyroiditis is present in nearly 40% of people with autoimmune atrophic gastritis. If you have Hashimoto's and persistent fatigue, low ferritin or low B12, this panel is a logical next step. It was built for people being evaluated for autoimmune gastritis or pernicious anemia, and a positive result is a reason to see a gastroenterologist, since the diagnosis is confirmed with stomach biopsies.

Included Tests
Intrinsic Factor Blocking Antibody
A highly specific blood test for pernicious anemia, the autoimmune cause of vitamin B12 deficiency.
Parietal Cell Antibody with Reflex to Titer
A sensitive blood test for autoimmune gastritis and pernicious anemia. If the screen is positive, the lab automatically measures how strongly.
Gastrin
Measures gastrin, the hormone that drives stomach acid. High gastrin with low acid is the pattern seen in autoimmune gastritis.
Fasting
Fasting Required
One or more tests in this panel require fasting for accurate results:
- Do not eat or drink anything except water for 8-12 hours before your blood draw.
- Water is allowed and encouraged - stay hydrated!
- Schedule your lab visit in the morning for convenience.
What This Panel Measures
Three markers of autoimmune gastritis: gastric parietal cell antibodies (a sensitive sign of the immune attack on acid-producing cells), intrinsic factor blocking antibodies (a highly specific sign of pernicious anemia) and fasting serum gastrin (which rises when the stomach can no longer make acid).
Who Should Get This Panel
People with low or borderline vitamin B12 or a raised methylmalonic acid; people with iron deficiency that has no clear explanation; people with Hashimoto's thyroiditis or other autoimmune thyroid disease, type 1 diabetes or another autoimmune condition who want to check for stomach involvement; people with a family history of pernicious anemia; and anyone a clinician is evaluating for autoimmune gastritis or pernicious anemia.

When to Get Tested
When low B12, low ferritin or an unexplained anemia needs a cause, or after an autoimmune thyroid diagnosis if symptoms or results suggest the stomach may be involved. Book a morning appointment after a 12-hour fast. If you receive B12 injections, schedule the draw at least one week after the last one.
Understanding Your Results
The three results are read together. All three normal makes autoimmune gastritis less likely. A positive parietal cell antibody screen (reported with a titer) with high fasting gastrin points toward autoimmune gastritis, especially if B12 or iron is low. A positive intrinsic factor blocking antibody is highly specific for pernicious anemia, while a negative one does not rule it out. Parietal cell antibodies alone, with normal gastrin, can occur in healthy people and often warrant watching rather than worry. High gastrin with negative antibodies has other common causes, such as acid-reducing medicines or H. pylori infection. JustLabs results are informational: review them with a clinician, who may check B12, methylmalonic acid, ferritin and a blood count, and refer you to a gastroenterologist, since the diagnosis is confirmed by stomach biopsy.
