Direct LDL Cholesterol Test

Direct LDL

Measures LDL cholesterol directly in the lab rather than estimating it from a formula. It is mainly useful when your triglycerides are high enough that a calculated LDL may be unreliable or cannot be reported.

LDL cholesterol is the main cholesterol-carrying particle linked to plaque build-up in the arteries, and it is the number most cholesterol treatment decisions focus on. On a standard lipid panel, LDL is usually not measured: it is calculated from total cholesterol, HDL cholesterol and triglycerides.

Calculation works well for most people. Quest uses the Martin-Hopkins method, which stays accurate even with triglycerides between 200 and 400 mg/dL. Above about 400 mg/dL, however, LDL may not be calculable, and that is where a direct measurement helps. Quest notes that direct LDL testing is reliable with triglycerides up to 1,000 mg/dL.

This test is not a better LDL test for everyone. If your triglycerides are normal, the LDL on a standard lipid panel is a sound number, and the lipid panel also gives you total cholesterol, HDL and triglycerides. Direct LDL makes sense if a previous lipid panel came back with very high triglycerides or without an LDL value, or if your clinician wants a measured LDL to follow over time.

For a deeper look at cardiovascular risk, apolipoprotein B and lipoprotein(a) are also available on JustLabs.

What's Measured

LDL Cholesterol, Direct

What This Test Measures

The concentration of low-density lipoprotein (LDL) cholesterol in serum, measured directly with an enzymatic method instead of being estimated from total cholesterol, HDL cholesterol and triglycerides.

Who Should Consider This Test

People whose triglycerides are high (especially above about 400 mg/dL) or whose previous lipid panel could not report a calculated LDL, and people whose clinician wants a measured LDL to track over time, for example while on cholesterol-lowering treatment. Most people starting out should choose the standard lipid panel instead.

When to Get Tested

When a previous lipid panel showed very high triglycerides or no LDL value, or on the schedule your clinician recommends for monitoring. Fasting is not needed for direct LDL on its own; if you are also ordering a separate triglycerides test, plan a 9-12 hour fast.

Understanding Your Results

Your report gives your LDL cholesterol in mg/dL. Quest's reference range for this test is under 100 mg/dL for adults 20 and older and under 110 mg/dL for people under 20, but the right target for you depends on your overall cardiovascular risk, which a clinician assesses. Higher LDL is associated with a greater risk of plaque build-up and coronary artery disease; it can reflect diet, weight and activity, smoking, inherited conditions such as familial hypercholesterolemia, an underactive thyroid, kidney disease, diabetes and some medications. A low LDL is usually not a concern on its own. Review your results with a clinician, especially if your LDL is high or you have other risk factors.

What to Expect

After placing your order, we send it straight to Quest Diagnostics — there is no consent form to sign and no partner email to wait for. Your requisition is usually ready to download from your dashboard within about an hour, and you can book your blood draw at any Quest Diagnostics location.

The blood draw typically takes just a few minutes. Results are usually available within 1-2 business days and will appear in your secure JustLabs dashboard.

Our results include reference ranges and easy-to-understand indicators showing whether your values are within normal limits.
