Complement C3 Blood Test

C3

Complement C3 is one of the immune system's complement proteins. Levels often drop when it is being used up by active autoimmune disease, which is why C3 is checked, together with C4 and anti-dsDNA, to evaluate and monitor lupus.

The complement system is a group of blood proteins that help the immune system fight infection and clear damaged cells and immune complexes. C3 and C4 are the most commonly measured individual complement proteins. When complement is heavily activated, as in some autoimmune diseases, these proteins are consumed faster than they are made and their levels fall.

Low complement is a key finding in lupus. The 2019 EULAR/ACR lupus classification criteria include 'complement proteins' as one of their immunologic domains, and MedlinePlus notes that active lupus often lowers C3 and C4. In people being treated for lupus, complement levels that rise back toward normal can suggest that treatment is working.

That is why C3 is rarely interpreted alone. Clinicians typically look at C3, C4 and anti-dsDNA together, alongside an ANA, blood counts, kidney function and a urinalysis. Low C3 can also be seen in certain kidney diseases (glomerulonephritis), cirrhosis, some infections and malnutrition.

This test uses a serum sample measured by immunoturbidimetry and requires no fasting. Results are informational and should be reviewed with a clinician.

What's Measured

Complement component C3

What This Test Measures

This test measures the concentration of complement component C3 in serum, in mg/dL, using an immunoturbidimetric method. For adults aged 15 to 80, Quest's reference range is 82-185 mg/dL for males and 83-193 mg/dL for females. Reference ranges are not established under age 1 or above age 80, and ranges vary between laboratories.

Who Should Consider This Test

People with a positive ANA or anti-dsDNA result who are being evaluated for lupus; people with diagnosed lupus or another autoimmune disease whose clinician uses complement levels to track activity and treatment response; and people whose clinician is investigating immune-complex kidney disease, unexplained inflammation or recurrent bacterial infections.

When to Get Tested

When lupus or another immune-complex disease is being evaluated, usually at the same draw as C4, anti-dsDNA and an ANA. In diagnosed lupus it is often repeated at intervals your rheumatologist sets, since a falling C3 can accompany a flare. No fasting or timing around meals is needed.

Understanding Your Results

A normal C3 level does not rule out autoimmune disease, but makes heavy complement consumption less likely at the time of the draw. A low C3 suggests complement is being used up, as can happen in active lupus, lupus nephritis and other forms of glomerulonephritis, or it may reflect reduced production, for example in cirrhosis or malnutrition, or rare inherited complement deficiencies. A high C3 is less specific; complement proteins can rise with inflammation, and MedlinePlus lists some cancers and ulcerative colitis among causes of increased complement. Interpretation depends on C4, anti-dsDNA, kidney tests and symptoms, so review your result with a clinician. JustLabs results are informational and are not a diagnosis.

What to Expect

After placing your order, we send it straight to Quest Diagnostics — there is no consent form to sign and no partner email to wait for. Your requisition is usually ready to download from your dashboard within about an hour, and you can book your blood draw at any Quest Diagnostics location.

The blood draw typically takes just a few minutes. Results are usually available within 1-5 business days and will appear in your secure JustLabs dashboard.

Our results include reference ranges and easy-to-understand indicators showing whether your values are within normal limits.

How much does the C3 test cost?

The C3 test costs $14 at JustLabs — $32 all-in, since the $18 lab & physician order fee is charged once per order, however many tests you add. The price is fixed before your blood draw; no insurance or doctor visit needed.
