17-OHP is the key test for telling nonclassic congenital adrenal hyperplasia apart from PCOS when someone has irregular cycles, acne, or unwanted hair growth.
17-Hydroxyprogesterone (17-OHP) is a steroid made by the adrenal glands as an intermediate step on the way to cortisol. When the enzyme 21-hydroxylase is partly deficient, that step backs up and 17-OHP accumulates while the body diverts the excess toward androgens.
That condition is nonclassic congenital adrenal hyperplasia (NCAH). It is far more common than the classic form and it looks very much like PCOS from the outside: irregular or absent periods, acne, and unwanted hair growth. The two are managed differently, so telling them apart matters.
Because the overlap with PCOS is so close, this test is usually ordered alongside testosterone, DHEA-S and SHBG rather than on its own.
This test is measured by liquid chromatography–tandem mass spectrometry (LC-MS/MS). Immunoassays for 17-OHP are known to cross-react with other steroids, which can produce misleadingly high results; LC-MS/MS separates 17-OHP from those look-alike molecules before measuring it. Endocrine Society guidance specifically favours an early-morning, mass-spectrometry measurement when congenital adrenal hyperplasia is being evaluated.

Collect in the EARLY MORNING, ideally before 8am — 17-OHP follows a strong daily rhythm and an afternoon sample can read misleadingly low. If you have periods, test during the follicular phase (roughly days 2-8), because 17-OHP rises naturally after ovulation. No fasting required.
Understanding Your Results
Early-morning follicular-phase levels below about 200 ng/dL make nonclassic congenital adrenal hyperplasia unlikely. Higher values are not a diagnosis on their own and are usually followed up with an ACTH stimulation test ordered by a clinician.
