Complete Thyroid
Our most complete thyroid panel, with parietal cell antibody and selenium added.
Complete Thyroid is Paloma Health's broadest panel. It includes everything in Thyroid + Nutrients, and adds parietal cell antibody and selenium.
The parietal cell antibody is the distinguishing test. Autoimmune thyroid disease and autoimmune gastritis often occur together, and autoimmune gastritis can reduce how well you absorb B12 and iron, and sometimes levothyroxine. Testing for this antibody alongside those nutrients can help explain low levels that keep coming back.

Included Tests
TSH (Thyroid Stimulating Hormone)
The TSH test is the gold standard for evaluating thyroid function. It's the first test doctors order when thyroid problems are suspected.
Free T4 (Thyroxine)
Free T4 measures the unbound, active form of thyroxine - the main hormone produced by your thyroid gland. Unlike total T4, free T4 is not affected by protein levels and gives a more accurate picture of thyroid function.
Free T3 (Triiodothyronine)
Free T3 measures the unbound, most active form of thyroid hormone. T3 is 3-4 times more potent than T4 and directly affects metabolism, heart rate, body temperature, and energy levels.
TPO & TG Antibodies
This panel tests for both major thyroid antibodies that indicate autoimmune thyroid disease, the most common cause of thyroid problems.
Reverse T3
Reverse T3 (rT3) is an inactive form of thyroid hormone produced when the body diverts T4 conversion away from active T3. Elevated levels may indicate stress, illness, or thyroid conversion problems.
Vitamin D, 25-Hydroxy
Vitamin D deficiency affects over 40% of Americans. This test measures your vitamin D status, essential for bone health, immune function, mood, and much more.
Ferritin
Ferritin is the best indicator of your body's iron stores. Low ferritin is one of the most common causes of fatigue, especially in women.
Vitamin B12
Vitamin B12 is essential for nerve function, DNA synthesis, red blood cell formation, and energy production. Deficiency is common and often undiagnosed.
Iron and Total Iron Binding Capacity
This test measures your circulating iron, your body's iron-carrying capacity (TIBC), and iron saturation percentage - giving you a complete picture of how your body handles iron.
Fasting
Parietal Cell Antibody with Reflex to Titer
Parietal cell antibodies target the cells that make stomach acid and intrinsic factor. Quest reports them in about 80% of people with pernicious anemia, which makes this a sensitive first look, though they also turn up in some healthy people. A positive screen is automatically followed by a titer on the same sample.
Selenium
Selenium is an essential trace mineral critical for thyroid hormone metabolism, antioxidant enzyme function, immune defense, and protection against oxidative stress.
Fasting Required
Fast for 8 to 12 hours before your visit. Water is fine.
What This Panel Measures
Thyroid: TSH, Free T4, Free T3, Reverse T3, and TPO and thyroglobulin antibodies.
Nutrients: ferritin, iron with TIBC and iron saturation, vitamin B12, and vitamin D.
Added in this panel:
- Parietal cell antibody: a marker of autoimmunity against the stomach cells that make acid and intrinsic factor, the protein needed to absorb B12. If the screen is positive, the lab automatically runs a titer, which is included in the price.
- Selenium: a trace mineral the thyroid uses in making and converting its hormones.
Who Should Get This Panel
This panel suits people with Hashimoto's or another autoimmune thyroid condition who have low B12 or iron that keeps returning, who need unusually high doses of levothyroxine, or who have a personal or family history of other autoimmune conditions.
If you mainly want to follow thyroid function, Thyroid Essentials is usually enough.

When to Get Tested
Consider this panel as a one-time, broader look when simpler testing has not explained your symptoms. The antibody and selenium results rarely need repeating on a routine schedule. Your clinician can advise which parts to recheck.
Test thyroid function 6 to 8 weeks after any dose change.
How to prepare
This panel requires fasting because of the iron test, so plan a morning draw.
- Thyroid medication: if you take levothyroxine or another thyroid medication, have your blood drawn before that day's dose and bring the dose with you. Keep taking your medication as prescribed otherwise.
- Biotin: high-dose biotin, common in hair, skin and nail supplements, can distort many of these lab results. Stop it at least 2 to 3 days before your draw.
- Supplements: iron can raise your iron result for a while after a dose, and selenium supplements can raise your selenium result. Ask your clinician whether to pause them before the draw.
Understanding Your Results
Your report shows Quest's reference range next to each result. The thyroid and nutrient results read as they do on any thyroid and nutrient panel: TSH usually moves in the opposite direction from thyroid hormone, antibodies point to autoimmunity, and low ferritin is a strong sign of low iron stores.
For the added tests:
- A positive parietal cell antibody raises the possibility of autoimmune gastritis, but does not establish it. The antibody also appears in some people without gastritis, more often with age. It is most meaningful alongside low B12 or iron, and confirming it usually involves a gastroenterologist.
- A negative result makes autoimmune gastritis less likely, but does not exclude it.
- Selenium reflects mostly recent intake. Both low and high levels matter, because too much selenium is harmful.
